You Found Open Penetrations in a Rated Wall

What counts as an unsealed penetration

A penetration is any item that passes through a fire-resistance-rated wall, floor or ceiling assembly — pipe, conduit, cable, cable tray, duct, sleeve, structural member. When the space around that item is not protected with an appropriate tested system, the assembly no longer performs the way its rating assumes.

The conditions that get cited most often:

  • Cable bundles passing through an oversized cored hole with open gaps
  • Sleeves installed but never sealed at the annular space
  • Abandoned penetrations left open after equipment was removed
  • Openings filled with insulation, spray foam or ordinary sealant instead of a tested system
  • Head-of-wall and joint conditions that were never addressed
  • Penetrations sealed on one side of a two-sided condition

Where they are usually found

Above corridor ceilings, in electrical and telecom rooms, in shafts and stairwells, at the top of demising walls between tenant spaces, and anywhere the building's systems cross a rated separation. They are almost never visible from the occupied space, which is why they surface during surveys, renovations and inspections rather than during normal operation.

Why buildings end up like this

In the overwhelming majority of cases, nobody did anything reckless. It accumulates:

  • A low-voltage vendor pulls new cable and drills through a wall
  • A tenant build-out adds plumbing through a floor
  • Equipment is replaced and the old penetration is abandoned
  • A repair opens an assembly and it is patched cosmetically, not to its rating
  • Nobody owns the record, so nobody knows which walls are rated

That last point is the root cause more often than any single trade. When a building has no current life-safety drawings identifying which walls are rated, the people drilling through them have no way of knowing that they should stop and call someone.

How a proper correction works

  1. Identify the assembly. The rating and construction of the wall or floor determines what systems are even eligible.
  2. Identify the penetrating item. Material, diameter, quantity, whether it is combustible, and how much space surrounds it.
  3. Match a tested and listed system. The installed condition needs to match a configuration that has actually been tested — not a general-purpose product applied by feel.
  4. Install and document. Photograph the condition before and after and record the location and the system used.

The most important thing to understand: the sealant is not the firestop. The firestop is the whole tested configuration. Two openings that look identical can require different treatment because the wall construction or the penetrating item is different.

Common questions

Can I just fill the gap with fire-rated caulk?

Not reliably. Firestopping is tested as a system — the assembly, the penetrating item and the sealing materials together in a specific configuration. Applying a product without matching a tested configuration is the most common reason a corrected opening gets cited again.

How do I know which of my walls are actually rated?

Life-safety drawings are the normal source, and many older buildings no longer have current ones. When they are missing or unreliable, establishing which separations are rated is part of what a survey or assessment produces.

We found a few open penetrations. Does that mean the whole building is a problem?

Not necessarily, but it is worth checking. The conditions that created one open penetration — cabling work, equipment changes, tenant build-outs — usually happened throughout the building, so isolated findings often indicate a pattern rather than an exception.

Is an abandoned, unused opening still a problem?

Yes. An opening through a rated assembly affects that assembly whether or not anything is currently running through it. Abandoned penetrations are one of the most frequently cited conditions.
