You Failed a Firestop Inspection. Now What?
A deficiency list is stressful, but it is usually solvable and it is almost always more organized than it looks. Here is how corrections normally work — and what an inspector will want to see when they come back.

First: this is normal, and it is fixable
Firestop deficiencies are one of the most commonly cited categories in existing commercial buildings. That is not because owners are negligent — it is because rated walls and floors get penetrated continuously over a building's life by cabling, plumbing, HVAC and tenant work, and the protection around those penetrations frequently does not get restored.
The result is that a barrier survey or annual inspection of a building that has been occupied for fifteen years can turn up dozens or hundreds of items. A long list is not necessarily a severe problem. It is often many small, repeatable repairs.
What an inspection failure actually asks of you
There are usually three separate obligations bundled into one letter:
- Correct the condition. Each cited opening, joint or damaged assembly has to be brought back to a protected condition using an appropriate tested and listed system for that specific configuration.
- Document what was done. This is the part owners underestimate. The inspector generally wants to know what was found, what was installed, where it was installed and that the installed condition matches a listed system. Photos and a location-by-location log are typical.
- Meet the deadline. Correction windows are set by whoever issued the notice. If the window is not realistic for the volume of work, that is usually a conversation to have early rather than after it lapses.
Why “just caulk it” fails re-inspection
Firestopping is tested as a system — the assembly, the item penetrating it and the materials sealing around it, in a specific configuration. Generic sealant applied to an opening is not a tested system, and an inspector who knows what they are looking at will cite it again. This is the single most common reason a building fails a second time.
A realistic sequence for getting this closed out
- Read the notice for its actual scope and deadline. Separate firestop items from other life-safety items, which are frequently mixed into the same report.
- Get the deficiency list into a usable format. Location, floor, type of penetration, assembly type. If the report is a scanned PDF with no structure, this step matters more than it sounds.
- Have a qualified firestop professional verify the field conditions. Reports are not always complete, and repair methods cannot be chosen responsibly from a spreadsheet alone.
- Confirm the correction approach before work starts. Including how the work will be documented for re-inspection.
- Execute and document location by location. Before/after photos and a log tied to the deficiency numbers.
- Schedule the re-inspection with the documentation in hand.
Mistakes that cost the most
- Sending in a general handyman or painter to seal openings
- Repairing everything with no photos, then having nothing to show at re-inspection
- Waiting until the last week of the correction window to get pricing
- Assuming the report captured everything — surveys frequently sample rather than cover

How long do I usually have to correct firestop deficiencies?
The correction window is set by whoever issued the notice — a fire marshal, an authority having jurisdiction, an insurer or a third-party surveyor — and it varies widely by jurisdiction and by severity. Read the notice itself for the deadline, and if the volume of work is not realistic within that window, raise it early rather than after it lapses.
Can my maintenance staff fix firestop deficiencies themselves?
Some owners do handle simple, repeated conditions in-house, but the installed condition still has to match a tested and listed system for that specific penetration and assembly, and it still has to be documented for re-inspection. That combination is why most inspection-driven corrections go to a firestop specialist.
Why did we fail again after we already sealed everything?
Almost always because the repair was not an installation of a tested firestop system, or because there was no documentation showing what was installed where. Generic sealant in an opening is a very common repeat citation.
Does the whole building have to be surveyed, or only the cited items?
That depends on the notice and the jurisdiction. Many surveys sample a portion of the building, which means the cited items may not be the full extent of the condition. A qualified professional reviewing the actual field conditions is what establishes the real scope.
